TREAT ME Now

Rapid response for adolescent substance use

Using a hub-and-spoke modeling, we now offer treatment to teens through TREAT ME Now with SUD challenges via phone intake, assessment and team support. The intake ‘HUB” will respond within 24 hours to the client/patient, family, Emergency Department, schools, corrections or other youth facilities. The assessment team includes an intake coordinator, clinician, case manager, operation lead, medical provider and if indicated a higher-level psychiatric provider. Triage team will transition youth to the appropriate setting: Emergency Department, residential, outpatient, primary care, case management, or community based medical withdrawal. Recovery support will be planned as treatment commences with guidance. Partnerships are established to secure proper transition to behavioral health hospital services, peer recovery coaching services, and primary care support. This model provides wrap around service access and ongoing primary care support.
The primary care offices are instrumental and offer a physical site for communication, support, and continuation of care needs.
